8D Facilitators

8D Facilitators can assist organisations in the delivery and administration of 8D Problem Solving Projects. Facilitators track the progress of the problem solving project and move the discovery of the root cause and implementation of counter measures forward. Facilitation assistance can in the form of face to face onsite presence or via web enabled work environments. The key is to follow the 8D process through all 8D phases and track data until the root cause is found, permanent correct action is implemented and steps for prevention have been completed.

Our facilitators provide guidance and delivery of the 8D process and ideally hand over to a role in the organisation for future 8D faciltiation once internal capability has been established. 8D Facilitation Process

The 8D Facilitation Process follow the 8D from D0 thorugh to the end of D8.

• Establishing the need for the 8D
• Ensure an Emergency Response Action (ERA) is in place
• Monitor ERA while in place
• Investigate and ensure data source allows for problem solving to occur
• Update any data managment requirement gaps related to the problem immediately
• Gather the correct team of SME's to solve the problem
• Gather and organise data as facts to allow problem solving to occur
• Develop and Implement an Interim Containment Action (ICA) based on exiting facts
• Remove ERA if required
• Monitor ICA while in place
• Continue Data Collection as required
• Develop and Test Root Cause Theories against data
• Validate data logic tested root cause theories through testing and validate root cause/causes
• Develop and Implement a Permanent Corrective Action (PCA)
• Remove ICA
• Investigate potential for recurrence of problem across the organisation
• Ensure all related documentation and data is updated to reflect the PCA
• Ensure the team is recognise in an appropriate manner.
